Weekly Great Vault: If you completed at least one Mythic+ dungeon the previous week, you will find a new Keystone in your Great Vault, typically one level lower than the highest key you completed.

Once you have a key, completing a dungeon within the time limit upgrades it to a higher level key for a different random dungeon. Failing the timer downgrades the key by one level but changes it to a different random dungeon.

How does the gear drop work in the end-of-dungeon chest?

The end-of-dungeon chest will drop one item per team with ilvl ranging from 684 to 707, depending on the keystone level. If you select the "Beating the timer" option, an additional item will be dropped. The "Increase your loot chance" option also maximizes your chance of getting gear.

While running keys higher than +10 will increase your Mythic+ rating and offer slightly higher ilvl gear from the end-of-dungeon chest, it does not further increase the item level offered in the Great Vault beyond what a +10 provides.
